You can download the questionnaire Performa (case record form) provided and fill it up as thoroughly as possible. Kindly post this duly completed Case record format to the address given below. Upon case analysis and study of the same, we will consult and prescribe the appropriate homoeopathic medicines, which in turn will be sent through courier or speed-post to the mailing address mentioned by you in the Performa.
